Dosage Coding Guidelines for Unlisted Drugs
In order to expedite claims processing and to eliminate the request for the Medication Administration Record (MAR), the exact dosage (i.e., mg, GM, mL) given to the patient is required when billing for physician-administered drugs. This dosage should be submitted in the appropriate drug quantity field of the electronic claim and not in the procedure code units. In addition, unlisted drugs do not have an assigned Healthcare Common Procedure Coding System (HCPCS) dosage, so using the free-formed note segment within the electronic claim can also be utilized to provide additional clarifying information that may be helpful in adjudicating the claim.
