Blue High Performance Network Providers

The Blue High Performance Network® (BlueHPN®) includes the following Blue Cross-contracted providers. You can use our Find A Doctor tool to check a provider’s BlueHPN status. More information about this network is available on our BlueHPN webpage and our BlueHPN FAQs.

Ambulance All ground and air ambulance providers regardless of location.
Ancillary Providers All preferred ancillary providers within the MSA. The following are included: audiologists, chiropractors, physical therapists, occupational therapists and speech language pathologists.
Ambulatory Surgery Centers (ASC) All ASCs within the MSA.
Dialysis Center All dialysis centers within the state of Alabama.
Durable Medical Equipment (DME) Suppliers All DME suppliers regardless of location.
Home Health, Home Infusion and Hospice Providers All Home Health, Home Infusion and Hospice providers within the state of Alabama.
Laboratories All Preferred Medical Laboratories regardless of location and all Preferred Physician Laboratories within the MSA.
Long-Term Care Facilities Specialty/free-standing long-term care facilities within the MSA.
Mental Health Providers All behavioral health providers within the state of Alabama. The following are included: Clinical Nurse Specialists, Psychiatrists, Clinical Psychologists, Nurse Practitioners, Licensed Clinical Social Workers, Licensed Professional Counselors and Behavior Analysts.
Mental Health Substance Abuse Inpatient Facilities Specialty/free-standing behavioral health facilities inside and outside of the MSA; and short-term acute care hospitals with behavioral health units within the MSA that meet BlueHPN designation criteria.
Rehabilitation Facilities Specialty/free-standing rehabilitation facilities within the MSA; and short-term acute care hospitals with inpatient rehabilitation units within the MSA that meet BlueHPN designation criteria.
Short-Term Acute Care Hospitals The following Short-Term Acute Care Hospitals are included:
•    Bibb Medical Center
•    St. Vincent's Birmingham
•    St. Vincent's Blount
•    St. Vincent's Chilton
•    St. Vincent's East
•    St. Vincent's St. Clair
Note: It is important that providers refer BlueHPN members to in-network hospitals to ensure they have benefits.
 
Skilled Nursing Facilities

Not covered
Specialty Hospitals The following Specialty Hospitals are included:
•    UAB Callahan Eye Hospital
•    Children’s of Alabama
•    Fox Army Health Center
•    Lyster Army Health Clinic
•    VA Medical Center – Birmingham
•    VA Medical Center – Tuscaloosa
•    VA Medical Center – Montgomery/Tuskegee
 
Transplant Facilities Blue Distinction Center for Transplant designated facilities regardless of location.
